4. You must fill in all fields that have a * next to it. |

5. If you purchase directly
from TYAN then please put your Company name directly in
the Company Name field. |

6. If you purchase from
any 3rd party sources (online, local stores, etc.) that do not
include directly
from TYAN then put
Distribution Customer
in the Company Name
